What is the purpose of your visit and what are you suffering from.
Is it your first examination? Have you been already treated in our clinic?

After the reservation of the first visit, please come to the reception desk with the following documents.
Identification card with photo (ID) or passport
Insurance card, non-insured clients need other ID card (driving license, etc. not necessary with photo)
Available medical documentation, Summary of up to now treatment
